From my experience this is a problem with database file. Some players work. Some not.

Need debug then. Get some IPs which not detected, check in database.
If it fine, need go next and check code for understand why it is not work.
If not fine, need search more fresh db.
Also some players show correctly. Some other players too. So I think base is outdated or broken or in wrong format after some records.

I've found the reason: the first given lookup server in server's IpToCountry.ini was "www.wharthog.com" which has not responded since mid 2021. IpToCountry should have tried the next given items in the list but it seems that does not work.

Seams like most dont respond so atm I only 1 left, is there some guide to set it up yourself since I have a server-machine?

looks like this release has files for you to setup your own server;

Note: there's nothing in the .logs to indicate which server IPToCountry is connected to or querying.
provided the sources inside attached file perhaps someone could output a few lines to the .log file indicating which server is being utilized and if any problems connecting
to query servers then output that info also to the .logs.

Yep. Recently I have added a link named test where it can be tested by web browser easily.

It seams that if the first server in line get a lag/hickup it dont jump to the next so it shows some other flag, so I created my own on the machine we rent (in Germany) and all works perfect so far.

Yep, a bug in IpToCountry, as I wrote above.

I think I found the error: switching to next server is only done when DNS resolving fails.
If there is already an IP address stored in IpToCountry.ini ("resolvedAddress") for that server, DNS resolving does not happen and next server is never taken.
Workaround: set all 4 variables resolvedAddress to an empty string and write protect IpToCountry.ini.

What are these for?
Do you mean var config string IPData[256];? These are cache entries for client's IP, hostname and country. Once determined, the entries stays in memory until next server restart/next map (not sure) regardless of a write protected INI file so that the performance impact should be neglectable.
