Two ports are needed:
---
Incoming port: 7777
Protocol: UDP
Forward to: (IP of destination host): 7777
---
Incoming port: 7778
Protocol: UDP
Forward to: (IP of destination host): 7778
---
(This applies only if you use the default ports 7777/7778 in your UT server.)

I thought you know the IP of the host your UT server is running on. ;o)
If that host is running Windows: start cmd.exe, type IPCONFIG and look for "IPv4-Adress". It might be something like "192.168.1.42".
If that host is running linux: open a console and type IFCONFIG and then as above.
Enter this IP value in the field "Server IP address" in your router. "Start Port" should be 7777, "End Port" 7778. You can restrict "Service Type" to UDP.
(All this is guessed by the screen shots you provided and assuming a simple network constellation - I may be wrong.)

If the host running your UT server gets its IP via DHCP, the IP may change after a some days. To avoid this, make a fixed entry of it (at the DHCP server, probably integrated in your router).

PS: Greetings to Detroit.

First calm down with double posts in 24 hours - USE EDIT BUTTON. Then...
I don't know if you have to specify External address but you have to specify port on which Internal LAN IP is routed - in CCTV we are forwarding 80 and over 30000 for connecting CCTV equipment. Internal address 192.168.0.xxx is address that has to be specified when a call on that port is coming through router. Also it is advisable a MAC specification for that address preventing to not being redistributed in other LAN machines. Aside, some routers at a moment of their existence might go retarded until you set DMZ for that internal address - then nothing will be filtered having ports open to connections - any - it's a little nasty this way.
If I'm not mistaking you must take in account MasterServer ports else your server will be stealth-invisible (as mine on purpose) for regular players.
Edit:
While you start whatever Win server you might input at command prompt "netstat" (query even help parameters). It shows which app is listening and where/active connections, etc. You can figure what UT is listening.

That's not in intention but I see a cascade of posts which admins are ignoring as usual, but later they jump up to clouds (called delay or lagged reaction). Else name of thread is How to port forward and it's exactly what I was pointing as example. Btw, right now I'm using a router but I keep the server open managed by a firewall. It would be advisable to know if your ISP is friendly at some points - because some ISP services are not that friendly with ports opened, just saying.
