I remember it vaguely being about something like this.

Design your image in Paint or Paintshop Pro etc and save the image as a .PCX

Find the relevant file in the UT directory and replace it with your file (rename your file the original files name and move the old one out, dont delete it incase you %£^$ up.)

Can anyone expand? :noidea

another thing you can do to change it is, go to your ut view settings then it should show someting like, show background or something like that, which will show a black background with Unreal Tournament in gold lettering, thats actually a map. Now what u can do is make a map, then in the level info under game type it should be UTintro, and something in the unreal.ini would have to be changed. This is what im concluding, i havent actually done this, but i think this is what u do. I actually downloaded a map awhile ago that was a background.

I actually just made a custom background just recently. Just made a night scene with brick walls, and a tree in the middle with moonlight shining down. U put a player start in and change the default game type to UTIntro, and just change the .ini to incorporate the new map. A problem i have is the camera is able to move, like having no gun and no hud, idk how they make the camera thingy not movable. But it begins where the city intro does, then when u press esc, the menu just comes up leaving the map in the background, once u esc the menu, u just continue moving...basically like starting a practice session and pressing escape and pausing

What the big surprise? Yeah, the background is always a flyby map, starting by CityIntro and ending in that map.
