How problem look:
You run on map with players or bots.
You step into lava or gasbag shoot you or fire hit you.
Suddenly you teleported to start and in hud messages appear text
"The server has prevented you from dying".
Why this happens:
Nexgen have some features for prevent team kill by damage and by push you into lava, acid and so on.
But this is not work as intended.
How fix:
Open nexgen console from admin, go to tabs:
Server > Settings > Nexgen
Set options (in right column) to zero:
Team kill damage protect time.
Team kill push protection time.
